Current RN registration with CRNM required.
Baccalaureate of Nursing preferred.
It is recommended that, at minimum, staff possess the Family & Friends category of CPR training.
Minimum 3-5 years clinical experience in managing chronic illness, especially heart failure and diabetes, required.
Fluency in English required, in French desired, and other languages an asset.
Minimum of 3-5 years’ experience in Emergency, Community health or a Medical Unit managing clients with chronic illnesses required.
Previous experience in a medical call centre environment preferred.
Previous experience in chronic disease management via telephone and Telehealth videoconferencing preferred.
Demonstrated advanced level computer-technical abilities and experience using McKesson Software Solutions and an EMR system preferred.
Demonstrated effective verbal and written communication skills required.
Advanced triage or assessment skills required.
Proficient in Outlook, Excel and Word required.
A satisfactory employment record is required.
