Description:

Under the supervision of the Patient Care Manager, and under the direction of the Registered Nurse, the Health Care Aide (HCA) acts as a member of the interdisciplinary health care team, assisting with patient/resident care and providing basic supportive services as delegated. The Health Care Aide demonstrates a commitment to the vision, values, goals and objectives of the region and Riverview Health Centre.

Qualifications:

Education:

• Grade X

• Health Care Aide Certificate from a recognized institution required.

Experience:

• 1 year previous experience in providing rehabilitative, long term care or chronic care to patients/residents is preferred

• Satisfactory employment record required

Knowledge:

• Knowledge of basic nursing care principles, techniques and procedures.

• Knowledge of Personal Health Information Act (PHIA), Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices and other relevant Workplace Safety and Health requirements/responsibilities.

• Knowledge of regional, site and work area policies and procedures.

• Knowledge of roles and responsibilities of members of the health care team.

Abilities and Skills:

• Skill in providing specialized and/or long term nursing care to patient/residents.

• Skill in application of appropriate techniques for patient/resident lifting, transferring and mobilization.

• Basic computer and keyboarding skills.

• Demonstrated effective verbal and written communication skills

• Ability to listen and comprehend effectively.

• Ability to select and use appropriate communication methods pertinent to the situation.

Responsibilities:

Patient/Resident Care/Clinical Practice

Implements the interdisciplinary plan of care through completing the following activities:

  • Understands and respects patients’/residents’ rights.
  • Treats patients/residents and families with courtesy and compassion.
  • Involves patients/residents and families in care decisions.
  • Promotes and assists patients/residents in maintaining activities of daily living, including meeting hygiene, grooming, nutrition and elimination needs.
  • Assists patients/residents to maintain adequate rest and activity levels.
  • Maintains established patient/resident oxygen therapy.
  • Promotes and maintains patient/resident safety.
  • Assists with constant care and/or close observation to assigned patients/residents when delegated.
  • Monitors wandering patients/residents as assigned.
  • Responds promptly to patient/resident calls, ensuring call bells are placed within patient/resident reach.
  • Transports patients/residents as required.
  • Participates in Emergency Response codes as required/directed.
  • Responds to the cognitive, emotional, social, spiritual and cultural needs of assigned patients/residents.

Non-Patient Care

  • Assists with obtaining, cleaning and maintaining equipment as directed.
  • Demonstrates a working knowledge of unit equipment and its correct operation, e.g. patient lifts and transfer devices.
  • Uses resources in a cost effective manner.

Communication

  • Responds appropriately to inquiries, relays messages and initiates appropriate action as directed.
  • Reports and records patient/resident care provided and changes observed within scope of position.
  • Establishes/fosters positive interactions and relationships with patients/residents, families and other members of the interdisciplinary health care team through use of appropriate communication and active listening techniques.
  • Adapts communication style to meet patient/resident needs.
  • Respects patient/resident confidentiality and right to privacy.
  • Communicates relevant information to the interdisciplinary health care team.
  • Reports and records unusual occurrences in accordance with Centre policies.
  • Observes and reports unusual behaviors of patients/visitors/staff on the unit.

improvements and participation in quality improvement activities as requested.

Other Duties

  • Maintains current knowledge of Emergency Preparedness Plans and responds

appropriately to Emergency Codes (Emergency Public Address Announcements)

  • Promotes achievement/maintenance of a clean and safe work environment

  • Uses equipment and materials safely and efficiently
  • Identifies and reports hazards in accordance with established guidelines
  • Handles hazardous and infectious waste properly

POSITION ELEMENTS

Environmental Conditions

  • Requires shift work and may be required to work extended hours.
  • May encounter environmental and personal hazards including potential sources of infection and aggressive or agitated patients/residents.
  • Position involves physical activities such as standing, lifting, transferring and transporting patients/residents and equipment.

Impact of Error

  • Inappropriate/insensitive communication, action or poor judgment may adversely affect patient/resident care or negatively impact the facility’s reputation.
  • Non-adherence to Centre policies and procedures may result in harm to individuals or self or damage to equipment and/or property.
