 Current CPR and First Aid, driver’s licence, with a preference for CNA certification in psychiatric/mental health nursing….

Shift Type: days with occasional evening shifts Reporting to the Clinical Manager of the Outpatient Services the Community Clinician will provide interventions for individuals with a serious mental illness living in a community setting and/or preparing to live in the community. These interventions and strategies involve the provision, coordination, and monitoring of complex psychosocial interventions to support individuals in their recovery. Key Accountabilities:

 Comprehensive mental health and psychosocial assessments, documentation including treatment and responses, assessment of activities of daily living and general health issues, interview and observe clients, family, other service providers

 Work collaboratively with the client, multidisciplinary team and community partners to develop, implement and evaluate collaborative plan of care; health teach and coordinate aspects of provision of support including medication. Communicate with stakeholders regarding client’s progress and any specific concerns related to wellness.

 Develop, implement, and evaluate therapeutic treatment strategies and crisis management strategies with clients, families, other service providers, provide crisis intervention and risk management strategies; provision of services to support Community Treatment Order legislation

 Support client rehabilitation and community integration in collaboration with community partners and primary care providers

 Liaise with community agencies/facilities to meet the needs of the population being served, provide information and promote and advocate for community based programs and services

 Develop, implement, facilitate and evaluate different treatment modalities ie group facilitation – CBT, DBT

 Participate in data collection, research and program evaluation projects, participation on committees

Requirements/Qualifications:

 Current registration in good standing as a Registered Nurse with the College of Nurses of Ontario, or Registered Social Worker with the Ontario College of Social Workers and Social Service Workers

 Current CPR and First Aid, driver’s licence, with a preference for CNA certification in psychiatric/mental health nursing  Recent mental health nursing or social work experience, with preference for community experience in a mental health facility or

service, including familiarity with the recovery philosophy, and trauma informed care

 Demonstrated knowledge of and/or experience with medication practices, side effects, and current treatment modalities within scope of practice

 Experience and training with non-crisis intervention techniques

 Experience developing and leading therapeutic groups

 Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results

 Demonstrated leadership, conflict resolution, group facilitation and organizational skills

 Proven written and verbal communication skills

 Proven ability to adapt readily to change

 Models and promotes core ethical practice, Waypoint Values, and reflects an optimistic and positive attitude

 Preference for proficiency in French/English language
