The Victoria Division of Family Practice is hiring one part-time contract Project Coordinator, PMH/TiC to work with our Patient Medical Home/ Primary Care Network and Transitions in Care teams.

Job Description

The Project Coordinator, PMH/TiC, works in collaboration with the Patient Medical Home/ Primary Care Network (PMH/PCN) and the Transitions in Care (TiC) program areas to facilitate network development, team based care initiatives and to improve processes that support patient-centred care. The Project Coordinator will develop, implement, and document quality improvement activities related to these project areas. The Project Coordinator will work with a diverse group of internal and external stakeholders including physicians, allied heath clinicians, health authority representatives, and other members of the community.

RESPONSIBILITIES

The Project Coordinator, PMH/TiC works collaboratively with a variety of stakeholders to support physician engagement, practice change, innovation, and improvement activities related to the development of the Patient Medical Home/Primary Care Network model and to improving Transitions in Care:

  • Engage a diverse group of partners and stakeholders, facilitating collaborative conversations to create a common agenda for primary care transformation
  • Develop new processes and tools and apply existing ones to gather evidence and evaluate the effectiveness of programs and projects
  • Support capacity building and improvement work using innovative and creative initiatives
  • Promote evidence-based practice with consideration of the local culture and context
  • Contribute to knowledge translation and reporting of evaluation data and PDSA cycle outcomes for various stakeholder groups; document quality improvement processes and changes
  • Coordinate meetings; develop and maintain a work-plan; prepare and submit monthly status reports; attend relevant Committee meetings
  • Apply QI methodologies and change management techniques to support practice change and promote spread and sustainability
  • Other coordination activities established in collaboration with leadership and the program teams, including occasional support work for other program areas within the Victoria Division of Family Practice

QUALIFICATIONS

Education and Experience

  • A level of education, training, and experience equivalent to a Master’s degree in a relevant discipline, with a minimum of two (2) years’ experience in a comparable role, ideally in a healthcare setting, OR, a level of education or training equivalent to a Bachelor’s degree in a relevant discipline, in addition to a minimum of five (5) years’ experience in a comparable role, ideally in a healthcare setting
  • Experience and/or professional development in the areas of quality improvement, process improvement, physician engagement, evidence-based design, project evaluation and/or another relevant area
  • Experience leading multi-stakeholder initiatives or processes

Abilities, Skills, and Knowledge

  • Ability to use relationship building and facilitation skills effectively in the application of quality improvement methodology and change theory
  • Ability to use creative and innovative approaches appropriate to the context, to influence thinking, attitudes, and behaviors
  • Ability to understand and process complex issues, anticipate and identify program challenges, and work collaboratively in a team environment to address them in a timely and tactful manner
  • Ability to work with key stakeholders to identify expectations and develop a common agenda for change
  • Ability to synthesize and summarize data for analysis
  • Essential skills include:

  • Demonstrated excellent in oral and written communications
  • Excellent computer skills with proficiency in Microsoft Office including Word, Excel, and PowerPoint
  • Database management skills including Client Relations Management Software, online survey design software, and advanced Excel skills would be an asset
  • Knowledge of the BC health care system, especially recent developments in primary care
  • Knowledge of care coordination and transitions in care would be an asset

  • The Project Coordinator will work independently (e.g., from a home office), and will provide equipment required to fulfill the duties of the contract.
  • The Project Coordinator will require a valid B.C. Driver’s License and use of a personal vehicle, as the position requires regular travel to meetings throughout Greater Victoria
  • The Project Coordinator will be required to attend evening or early-morning meetings

TERM, HOURS, AND COMPENSATION

This is a part time contract commencing September 4, 2018 and ending March 31, 2019, with possibility of extension pending continued funding. The contract averages 30-40 hours/week, at a rate of $30-35/hour +GST, commensurate with educational level, experience, and skills.
