The Victoria Division of Family Practice is hiring one part-time contract Project Coordinator, PMH/TiC to work with our Patient Medical Home/ Primary Care Network and Transitions in Care teams. Job Description The Project Coordinator, PMH/TiC, works in collaboration with the Patient Medical Home/ Primary Care Network (PMH/PCN) and the Transitions in Care (TiC) program areas to facilitate network development, team based care initiatives and to improve processes that support patient-centred care. The Project Coordinator will develop, implement, and document quality improvement activities related to these project areas. The Project Coordinator will work with a diverse group of internal and external stakeholders including physicians, allied heath clinicians, health authority representatives, and other members of the community. RESPONSIBILITIES The Project Coordinator, PMH/TiC works collaboratively with a variety of stakeholders to support physician engagement, practice change, innovation, and improvement activities related to the development of the Patient Medical Home/Primary Care Network model and to improving Transitions in Care: QUALIFICATIONS Education and Experience Abilities, Skills, and Knowledge * TERM, HOURS, AND COMPENSATION This is a part time contract commencing September 4, 2018 and ending March 31, 2019, with possibility of extension pending continued funding. The contract averages 30-40 hours/week, at a rate of $30-35/hour +GST, commensurate with educational level, experience, and skills.
