POSITION SUMMARY:

As Change Management Site Coordinator for TransForm the role will support adoption of ehealth solutions in Erie St. Clair (ESC), Local Health Integration Network (LHIN). Responsible for ensuring standardized change management documentation and processes are adhered to for all implementations across the ESC LHIN.

This position is responsible for working with the Change Management Team to ensure the successful adoption of deliverables across the ESC LHIN.

ACCOUNTABILITIES:

  • Works with the project team and the end users (physicians/hospitals/NP’s/clinical office staff) to implement the change management and adoption strategy
  • Work with the project team to manage changes and impacts through assessments, awareness, process review/gap analysis
  • Follow a set of comprehensive standardized documentation and change management process across the ESC LHIN
  • Complete change management assessments, and change management strategy plans
  • Assist with the coordination of stakeholder communications content creation and audience analysis
  • Work with TransForm IT/IM staff and hospital resources to build test plans and coordinate testing
  • Identify, analyze, prepare risk mitigation tactics
  • Directly report to the Change Management Lead; provide regular updates on new and upcoming implementations and outstanding support issues
  • Coordinating implementations with the end users across different healthcare sectors
  • Adhere to pre-implementation rollout strategy by collecting necessary agreements, holding stakeholder engagement sessions, completion of readiness assessments and pre-implementation surveys
  • Offer and provide onsite training (direct or train-the-trainer) based on the project requirements
  • Provide ongoing support with any unresolved issues and/or implementation challenges
  • Continuous learning of new Electronic Medical Record and Electronic Healthcare Record systems and software updates as they become available
  • Coordinate with EMR vendor for any additional EMR training requirements
  • Support site technical resources, provides quality assurance testing
  • Coordinate EMR vendor upgrades or patches and validate the work being done
  • Work with local user groups to confirm & document report receipt & acceptance as needed
  • Coordinate post implementation follow up to confirm acceptance/adoption of solution
  • Track and monitor implementations and log any issues in a common portal
  • Provide lessons learned for future use and provide to Change Management Lead
  • Identify successes and challenges in adoption by monitoring feedback from users with regards to gaps/resistance in process; relay to the Change Management Lead/project team.
  • Participate and contribute to regional working groups as required. Share best practices and lessons learned with regional counterparts.

EDUCATION & EXPERIENCE:

  • Minimum 2 year College Diploma
  • Minimum 3 years change management experience
  • Demonstrated experience in developing and delivering change management and adoption programs including communications, training and adoption strategies
  • Demonstrated experience in building test scripts and executing software testing
  • Working in a human resource, communications or organizational development role
  • Knowledge of latest theories, models and strategies for assessing and leading organizational change, including documenting and design of business processes
  • Demonstrated ability to understand the inter-relationship between business processes and technology tools
  • Experience in defining the impact that change will have on staff/management/customers
  • Demonstrated knowledge of and experience in the healthcare sector
  • Strong interpersonal skills and the ability to handle difficult situations with tact and diplomacy
  • Strong skills with computer systems including all Microsoft offices tools
  • Demonstrated ability to quickly adapt and/or learn new system functionality and new technology as a user
  • Previous working knowledge of an EMR system (working knowledge of multiple EMR systems a strong asset)
  • Ability to manage time/heavy work load effectively
  • Strong analytic and decision making skills
  • Ability to work independently as well as within a team
