Position Description

SUMMARY

The Program Manager is an instrumental role in SHARP’s multi-disciplinary team to coordinate care and services including overseeing all aspects of service implementation and delivery for individuals living with HIV so they may lead healthier lives and achieve self-sufficiency if possible. The clients served by the SHARP Foundation have complex health and social profiles with co-morbidities which may include two or more of the following: Hepatitis C, addictions, mental health issues, physical disabilities, brain injuries, histories of abuse, homelessness and criminalization.

KEY ACCOUNTABILITIES

  • This position reports to the Executive Director.
  • The incumbent works closely with SHARPs multi-disciplinary Case/Care Team.

RESPONSIBILITES

Client Service and Program Delivery

  • Manage and supervise all operational activities of five scattered site programs in accordance with agency mission, policies and procedures, and funding standards in conjunction with Executive Director.
  • Analyze trends in the program, identifying issues and recommending solutions to the Executive Director.
    • Assist with proposal development and fundraising activities for the organization.
    • Ensure that all program goals and objectives are maintained and developed.
  • Develop and implement surveys, focus groups and other evaluation methods to monitor the efficacy of the Foundation’s programs and services and identify areas of improvement; assist the executive director with analysis of data.
  • Support collaboration with community agencies and partners regarding the needs of the client and to expand the potential of services.
  • Evaluate effectiveness of care plan and progress made by client, participate in client treatment planning and case review with case management team.
  • Support creation of forms and records to assist with the documental activities of programs.
  • Facilitate quarterly staff meetings to sustain effective communication and liaises with Health Manager regarding the planning and development of staff meetings.
  • Compile, maintain and report monthly, quarterly and annual program statistics.

Agency Operations

  • Coordinate the delivery of services among different program activities to increase effectiveness and efficiency with the Executive Director and Program Leads.
  • Provide training and support to staff in areas related to ensuring compliance with civic, provincial and national standards for supported living facilities maintaining the policies and procedures of the organization, using client management information systems, assist health manager with training on effective client support.
  • Assist the Executive Director with other operational tasks when required.

PROFESSIONAL COMPETENCIES

To succeed in this position the incumbent must have the following competencies and characteristics:

  • Excellent communication skills (writing, listening, speaking) at all levels (government reporting, probation/correctional personnel, health professionals, clients, etc.).
  • Experience in evaluation, analysis and narrative report creation.
  • Exhibit a strong network within the Calgary non-profit sectors.
  • Strong knowledge and experience working with marginalized populations.
  • Strong client focus – understanding of client self-determination, keeping the client engaged and participating in their own well-being.
  • Is genuinely committed to the organizational mission, values and vision.
  • A positive attitude, creativity, and flexibility.
  • Non-judgemental.
  • Strong communicator with ability to listen, share and support improvement in meeting client needs.

Qualifications/Knowledge:

  • Diploma or degree in Non-Profit Management and/or a minimum of three years management experience.
  • Strong interpersonal skills, adaptability, program planning experience and administrative abilities.
  • Strong knowledge of mental health and substance use from a harm reduction perspective.
  • Understanding of HIV/AIDS including GIPA/MIPA, stigma, discrimination, etc.
  • Knowledge of community health, justice and homeless issues and services sector.

Working Conditions:

  • Occasional evening and weekend hours may be required
