The St. Michael’s Hospital, Department of Family and Community Medicine operates a progressive community based Academic Family Health Team. With collectively over 60 physicians and nurse practitioners, our team is committed to providing comprehensive interdisciplinary patient care. Over the next 3 years, we expect our patient volume to grow as we expand our clinic services into the Regent Park area.

The Health Promoter will focus on interventions to assist low income clients of the St. Michael’s Family health Team improve their income and reduce the health impacts of living at low income.
DUTIES & RESPONSIBILITIES:
- Provides one-to-one case management support and ongoing follow-up to diverse low-income patients and families regarding their income security
- Conducts outreach to low income patients of the SMH Family Health Team, in collaboration with team health providers
- Develops and implements targeted information self-help sessions such as; banking basics, income tax submissions, social service forms etc. for the targeted population
- Develops and maintains up-to-date detailed knowledge of financial issues and financial services affecting low-income people
- Assists individual low income patients and families with income security-focused interventions
- Liaises with external organizations, including community social support agencies, legal aid clinics, homeless support agencies, advocacy groups and agencies focused on vulnerable sub-populations to develop referral pathways and supports for patients, with a focus on empowering patients to become change agents
- Develops the capacity of Family Health Team physicians and staff to identify low income patients and address income security
- Creates educational sessions and communications for physicians (CME) and other FHT staff (CPD), as well as for health provider trainees.
- Develops tools to assist in the identification and intervention of low income as a health risk
- Assesses the FHT’s capacity to address poverty as a health risk by developing a scorecard or measureable indicators (e.g. ability to identify clients, presence of team resources, presence of and links to community resources), including an annual report on progress in meeting those indicators
- Works with the program advisory committee to develop a high impact position within the family health team that is replicable and sustainable. This will include at least monthly meetings with the teams
- Develops a template for this position that can be used by other health teams to create similar positions
- Provides advice regarding the design of evaluation protocols
- Participates in evaluation data collection as needed
- Graduate degree from a recognized university in health promotion, public health, social science, or a related discipline
- Experience and knowledge of community organizing techniques, and program design, implementation and evaluation
- Knowledge of issues faced by underprivileged communities and the interface between income security and health
- Demonstrated commitment to and knowledge of community-based health care and resources
- Demonstrated ability to work effectively in a multi-disciplinary team environment
- Ability to work with diverse stakeholders
- Demonstrable knowledge of financial matters affecting low-income people, including: federal and provincial income security programs and policies, the Canadian and Ontario tax systems, access to financial services, assets and savings vehicles, and other income support programs such as WSIB and private insurance companies. The ability to develop and maintain up to date knowledge in these areas.
- Willingness to be trained in detailed financial matters and to remain current.
- Excellent verbal and written communication. Listening, organizational, multi-tasking and problem-solving skills.
- Ability to take initiative and demonstrate flexibility and advanced problem solving skills, in order to work with complex client situations and to enthusiastically develop a unique health team position.
- Experience in providing one-on-one supports (including assessment, problem-solving) to individuals, with diversity in ethnicity/language, income, education, ability, and mental health status.
- Experience building relationships with community partners that are outside the health care system.
- Experience in the development of educational materials targeted at health providers and patients.
- Experience with data collection, research and quality improvement
