Under the supervision of the Regional Manager- Access & Intensive Case Management, the Community Mental Health Intensive Case Manager Worker offers recovery orientated, community-based, rehabilitation, and supports to individuals living with a psychotic disorder and experiencing a severe and persistent functional impairment that limits major life activities. The case manager collaborates with other systems, organizations and individuals to ensure clients have access to a well-coordinated service system: assists the client and family to prioritize and focus on achieving optimal health, incorporating a psychosocial rehabilitation recovery focused model; provides comprehensive assessments, interventions and therapeutic support to individuals and their families; provides education and prevention material to individuals, families and the community at large. Services are aimed at enabling individuals to regain hope, self determination and empowerment in their recovery journey. Active license and registration to practice as required by professional association and Provincial legislation
Qualifications
· Requires a valid Class 5 driver’s license, an all purpose insured vehicle and liability insurance of at least $1,000,000.00
· Certificate in Applied Suicide Intervention Skills Training (ASIST) or (prepared to acquire)
· Knowledge of Psychosocial Rehabilitation and Recovery principles
· Experience providing Psychosocial Assessments, mental status exams and evidence-informed interventions
· Ability to support and implement recovery oriented practices of hope and optimism throughout the organizational culture
· Demonstrated ability to meet the physical and mental demands of the job
· Demonstrated ability to respect confidentiality including paper, electronic formats and other mediums
· Excellent organizational, prioritization, problem solving skills, detail-oriented and well developed communication skills
· Good work and attendance record
· Demonstrated ability to work within Respectful Workplace policies and guidelines
· Demonstrated ability to work collaboratively within a multi-disciplinary team and across service sectors
