Job Summary: Reporting to the Community Team Manager, the Clinical Coordinator combines knowledge of case management, supervisory and clinical nursing skills to effectively assess and coordinate client care in the nursing department.

Experience: Qualifications: Current Registration with the SRNA Degree from an approved School of Nursing Willingness to maintain or obtain required competencies in practice Demonstrated understanding of case management as part of the service delivery approach Self-motivated and self-directed Three years’ experience in community nursing required A minimum of one to two years in a hospital setting required Experience in Home Care Intake preferred Computer literacy in email, word documents and web search. Working knowledge of the Procura program and RAI-HC assessment tool preferred Ability to enter data reliably and efficiently through keyboarding Valid Saskatchewan Driver’s License and access to a reliable vehicle Ability to travel extensively and drive on rural roads Knowledge, Skills & Abilities: Team player with superior written, verbal and organizational skills Excellent interpersonal communication skills Requires ability to work independently with minimum supervision Requires skills to manage conflict There are regular requirements to meet deadlines, to enable client needs be met May need to deal with emotionally charged situations Utilizes case management strategies to develop the client care plan Sound knowledge of the nursing process Works within established policies and procedures Comprehensive knowledge of SRNA’s standards for nursing practice utilizing clinical pathways and evidence based care. Strong assessment skills and ability to identify unmet needs (clinically and socially) Strong problem solving skills and knowledge of available resources Makes judgments on the basis of critical thinking in order to creatively solve problems in an open and resourceful manner Interacts with clients, families and staff in a tactful, meaningful way Ability to handle several complex issues concurrently, while maintaining attention to detail Must be a client advocate Must be culturally sensitive when dealing with multi-cultural clients and co-workers Must be well organized and able to meet deadlines Ability to assist with policy and procedure development and quality assurance initiatives Demonstrates ability to work in a team successfully promoting collaborative practice with other professionals Demonstrates commitment to professional growth

Qualifications, Knowledge, and Skills: Required Qualifications
Bachelor Degree in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Valid Class 5 drivers license
