AboutMosaic Primary Care Network
Mosaic Primary Care Network (MPCN)is a group of family and allied health professionals in East Calgarycollaborating with Alberta Health Services – Calgary Zone to deliver the bestpossible primary care.

The MPCN brings together physiciansand other primary health care providers to achieve efficiencies, and assess andmeet patient needs and cooperate on special programs that serve the catchmentarea. The MPCN has established a teamenvironment where individuals are encouraged to take initiative, be creativeand contribute to ongoing decision making.

PositionSummary
The PhysicianLiaison (PL) is the primary communications connection between operations andthe membership base. A natural relationshipbuilder, the PL effectively builds trust through familiarizing him/herself withthe physician member clinic practices and promotes Mosaic PCN programs andservices to address practice concerns and support positive patient outcomes.
The PL provides anecdotal feedback tooperations regarding how well a program or service is being operationalized,barriers to implementation experienced by the membership, and suggestions forimprovement. The PL will work collaborativelywith operations to develop strategies for service delivery improvements and toaddress issues in an appropriate and timely manner.

Responsibilities

Primary responsibilities of thisposition include:
RelationshipBuilding PromoteMosaic PCN programs and services to physician members so that they canappropriately serve Mosaic PCN patient population Collaboratewith Mosaic PCN clinical operations to address physician concerns and needs tomaximize patient outcomes Interactwith physician membership to gather informal/anecdotal feedback for improvementof current services and development of new programs Maintaincollaborative team relationship with colleagues to effectively contribute toMosaic PCN’s goals

BusinessDevelopment Developand implement a physician engagement strategy to build relationships withphysicians within a defined geographic area Establishrelationships with healthcare service providers to increase referral sources Informand influence physicians regarding the features and benefits of Mosaic PCNmembership, PCN strategy, and the Provincial primary care strategy Developstrategies to overcome membership access and engagement barriers

QualityImprovement Collaboratewith the evaluation team to develop and review membership surveys, includingproviding contextual interpretation and situational follow up Identifyand support opportunities for membership engagement and issue resolution

Communications Communicateappropriate usage of Mosaic PCN forms as they relate to the physicianmembership Collaboratewith communications to develop and disseminate communications materials andmessaging as appropriate Collaboratewith various programs and services on special projects as appropriate

Other duties as assigned

Qualifications
Education& Licenses Bachelordegree, preferably in business (e.g. marketing), health care administration, orrelated field

Experience Minimumof five years’ experience in primary health care setting Educationand/or experience in change management Experiencein sales and/or communications

Knowledge,Skills & Abilities Abilityto work independently and be self driven Strongrelationship skills and ability to balance conflicting priorities Abilityto prioritize and make effective decisions on critical deadline driven projects Professionalwritten and oral communication skills Stronganalytical and problem solving skills Demonstratedability to work in a constantly changing environment Positive,enthusiastic and professional demeanour Excellentknowledge of Microsoft Office, particularly Word, Excel, and Outlook, with aworking knowledge of PowerPoint Abilityto quickly learn new information and technology and apply to a work setting Facilitationskills with the ability to influence behaviour

Conditionsof Employment Ability to sit,stand, and lift/ carry weight of approximately 15-25 lbs Evening work isrequired, weekends may be required Daily travelwithin NE and SE Calgary is required Minimum Class 5driver’s license, access to a reliable vehicle, and adequate insurance isrequired As the positionis mobile, adequate home storage/office space for drop down and preparationwill be required Successfulapplicants must provide proof of qualifications and a current policeinformation check (PIC) including vulnerable search at the applicant’sexpense. The results of the PIC mayalter or revoke any offer made by Mosaic PCN.
