Job Title : Follow Up Support Worker

Based at : Herb Jamieson Centre, Downtown

Reports to : Rapid Exit Coordinator

Hours of work : 40 hrs/weekly

To assist newly housed clients in maintaining their housing by developing structures and supports to enable a healthy re integration into society through encouraging and facilitating linkages to treatment facilities and services.

Establish rapport with client and promote advocacy

Provide assistance for clients in: home insurance, securing identification, bank accounts,

Linking clients with relevant community resources and support systems

Appropriate referrals to medical and psychosocial services.

Providing Life-Skills Education through modelling, mentorship and coaching

Liaison with landlords to ensure client’s compliance on matters relating to rental charges and established housing protocols.

Attending to clients’ needs through visitation, assisting with client’s transportation needs.

Tracking all work with participants through case notes and completing assessments on Efforts to Outcomes database.

Attending team meetings for collaboration; problem solving/brainstorming.

Preparing clients for eventual graduation from the program.

Reporting to Team Lead.

A degree or diploma in Social Work, Christian Counseling or other related discipline, and/or relevant experience in the field.

Proficiency in MS Office: knowledge of Excel, word processing and database softwares.

Experience in counseling and Life-Skills Education

Good interpersonal/Team building Skills

Skilled in Self-management

Knowledge and application of related ethical practices and standards

Class 5 Driver’s License with good driving record, use of personal vehicle and $1 millon insurance coverage.

Standar First Aid and CPR, Medication Admnistration, ASIST traning and Non-Violent Crisis Intervention training.

Security Clearance and Intervention Record Check.

Must be legally entitled to work in Canada on a full-time basis.

Able to provide Criminal Record Check and Vulnerable Sector Search

A letter of reference from a ministry or church is an asset
