PROGRAM AREA: Community Health EMPLOYMENT STATUS: One year contract (renewable) HOURS/WEEK: 35 hours per week (full time)

Nurturing Neighbourhoods is a collaborative partnership to increase the ability of families, neighbourhoods and communities to improve the safety and well being of children. All families have challenges. When those challenges do not get the support they need, sometimes serious problems occur, putting children at risk of harm. Using a population health and social determinants of health lens, this initiative seeks to engage families, communities and service providers to find the most optimal solutions. We want to build networks, resources and supports for families before children are at risk. We want to help families when problems are emerging – to anticipate and prevent those problems from harming kids in the first place. The program offers one-on-one and peer support building on existing community assets to create vibrant, resilient, healthy communities.

The Parent Outreach Worker will work with Nurturing Neighborhoods and will be located in a priority neighborhood which will be accessible to all community members who are caregivers to children. The overall objective of this position is to provide early help to children and families whose problems, if left without help, may grow and have irreparable impact on child outcomes.

Job Duties:
Build and implement an outreach plan to families in the community (eg. Sharing information, assisting with formal and informal referrals, accompanying/supporting access to community programs, supporting families to identify and prioritize issues, facilitate relationship building)
Coordinate neighborhood services with other agency partners
Build relationships with community health and social service providers
Respond to parents who identify that they need help with family challenges including but not limited to: health, housing; income; food insecurity; domestic/family violence; employment, parenting; and child behaviour.
Help parents navigate extensive community agencies and community support networks
Liaise with and advocate where necessary with partner agencies.
Promote community dialogue and discussion regarding benefits of early help strategies
Counsel, interpret, problem solve, help resolve conflicts, explain and exchange information with caregivers, parents, and children
Administrative functions: documentation, scheduling, arranging community meetings, attend internal and external meetings as required.

Qualifications:
Post-secondary education in a related field: Community or Social Services, Community Development or equivalent
3 Years outreach, counseling experience with vulnerable populations; experience working or volunteering in both a community and social service setting required
Comfort level and knowledgeable about working on issues related to poverty, inequality, child and family issues, child protection, addictions, mental health, family violence and positive parenting and family functioning
Excellent knowledge of community resources, supports and services in Guelph and Wellington County
Solid family assessment and advocacy skills
Experience with and understanding of strategies that support behavior change with vulnerable individuals/families.
Knowledge of the Social Determinants of Health
Excellent oral and written communication skills
Demonstrated ability to work as part of an interdisciplinary team
You must have a valid driver’s license and access to a vehicle
