Position Highlights

Bring your demonstrated knowledge of Ambulatory Care Services, principles and standards of practice to a dynamic and rewarding role providing leadership to all processes and systems related to patient referrals to acute inpatient and outpatient clinical services.

Utilize your extensive knowledge of the health care systems as it pertains to community and ambulatory services and the relationship with other clinical programs and services.

The Ambulatory Care Coordinator is responsible for:
• Leading the planning, implementation and evaluation of sustainable access to Ambulatory Care Services and ensure the needs of the target population are meet within Burnaby Health Services area and its Community.

• Providing leadership to processes and systems related to patient referrals and community in-reach and outreach clinical services.

• Working with physicians, managers and clinical staff at the Fraser Health sites to facilitate patient referral and transfers to Fraser Health and facilities outside of the organization.

• Supporting the Director, clinical operations, managers, physicians and clinical staff to review ambulatory service referrals and identify, reduce and remove barriers to patient flow and recommend and implement changes in patient referrals, to facilitate effective and appropriate ambulatory service delivery consistent with FH policies, strategic objectives, plans and vision.

As the successful candidate, you have a Bachelor’s degree in a health-related discipline, supplemented with five (5) to seven (7) years’ recent related clinical experience in a complex healthcare environment including two (2) years’ experience in a leadership/management role, or an equivalent combination of education, training and experience.

This position is a permanent full-time located at Burnaby Hospital.
