Role Description: The Community Disability Support Worker is the front-line position of a support team. They work most directly with individuals with disabilities. They are part of a team that ensures individuals have quality of life in areas including: daily living, recreation and leisure and community inclusion. The Support Worker reports to the Assistant Manager and Service Manager.

Responsibilities:

Direct Service

  • Follow through with arrangements that were made for individuals to achieve full potential in employment, community, social, residential environments and life pursuits. Act as an advocate for individuals when needed. Ensure individuals are satisfied and daily needs are met. Provide Personal Care in a dignified and private manner. Administer Medication Staff communicate any health concerns.
  • Attend/Follow up with medical appointments as designated by team leader. Administer First Aid.
  • Ensure home maintenance e.g. cleaning, laundry, decorating, etc. is completed while encouraging individuals to participate to their fullest ability.

Day Time Activity

  • Vocational: support clients in their individual goals to participate in productive or skill development by determining their vocational interests.
  • Recreational: assist individuals to discover and participate in leisure time activities.

Safety

  • Individuals are kept safe.
  • Individuals are safe from physical harm.
  • Staff follow procedures outlined in behavioral programming that help individuals with behaviors of concern.
  • Individuals are free from abuse.

Qualifications:

  • Minimum of a high school diploma and experience with complex behaviors is an asset.
  • All Entrust employees must clear a Criminal Background Check, CYIM, and have First Aid/CPR.
