One (1) Temporary Part-Time position
from June 6, 2016 – March 31, 2017
The Health Home and Community Program (HHC) is seeking a part-time ADL Assistant to join our team. HHC program is an interdisciplinary team providing on site support to tenants living at 220 Oak St.
1. Primary responsibilities are as follows;
- Assists with the development of client support/Goal plans
- provides input into each clients support/Goal plan
- Shares responsibility with the team for implementing supports that assist clients in pursuing/achieving their goals
- Assumes a primary role in assisting clients with Activities of Daily Living
2. Directly supports clients with Activities of Daily living such as;
- Assisting clients with Laundry routines
- establishing and or maintaining clients cleaning routines
- maintaining and cleaning clients units
- other supports with ADL’S as identified by the Supervisor or the team
3. Supports the maintenance of safe/clean environment
- Assisting with any pest control/eradication efforts, as required
- Continually assessing the environment for safety and sevurity issues and recommending action to address any deficiencies.
4. Coordinates and supports other activites with the Team including planned outings and group activities.
5. Dcomentation and Data entry
- Documents all client contacts/interactions in the appropriate client chart according to Cota’s documentation guidelines
- Enters data relating to client interactions into Cota’s database system according to data collection guidelines
- Assists with other data collection activites within the program, as determined by the supervisor or team
6. Participates in Team meetings and provides input and ideas for improving services, efficienceis and ongoing program planning.
Working Conditions
- Supporting/visiting clients in their apartments at at 220 Oak St.
- Supporting clients in the drop in
- Comfortable working with clients with a broad range of needs
The successful canditate requires;
- Certification as a PSW or Health Care Aide. Higher level of post secondary education will be considered an asset
- Relevant experience in assisting clients with activities of daily living, household management including the potential of Hoarding issues
- Direct or transferable experience suitable to working effectively with the relevant population.
- Good interpersonal skills
- Good judgement and ability to work independantly as well as coorperatively with others
- Non judgemental attitude
- Proven record – consistent, reliable and flexible
- Ability to recognize safety issues and follow safe work practices
- Demonstrated client-centred and customer focussed approach
- Abiltiy to problem solve and make recommendation to resolve issues
- Effective team player
- Sensitive to challenges experienced by marginalized population groups such as poverty access issues, mental health and addictions.
Judgement
- Ability to identify client risk factors and coordinate appropriate follow-up
- Respectful and aware of client – worker boundaries
- Awareness of safety issues with clients
Hours of work:
18 hours wk which includes 3 days, 6 hours a day each.
