The South West Community Care Access Centre (CCAC) serves 60,000 people each year, across a vast region from Tobermory in the north to Long Point and Port Glasgow in the south. Our role is to get people the home and community care they need to stay well, heal at home and stay safely in their homes longer. We also help people transition through the system and to other living arrangements. We do it by working in partnership with patients, families, providers, community organizations, and others.

Connecting South West Ontario (cSWO) is funded by eHealth Ontario with the goal to implement a Regional eHealth Program that will make an individual’s health information from across the continuum of care available in a timely and secure fashion at any point of care. The cSWO Program is foundational to eHealth Ontario’s commitment to integrate electronic health information for all Ontarians.

London Health Sciences Centre (LHSC) is the delivery partner to eHealth Ontario, with accountability for the cSWO Program Management. There are four Change Management and Adoption Delivery Partners (CM&A DP) that are responsible for the deployment of cSWO Program services. Their role is to work with local health service providers to facilitate the business and technical adoption of cSWO electronic health regional Program solutions and standards into the regular delivery of care.

In the South West LHIN, the cSWO Program is being delivered by the South West CCAC. This exciting multi-year Program includes the planning, implementation, adoption and evaluation of key components of the electronic health record and is focused on engagement with local health service providers to leverage existing provincial, regional and local solutions, including the Regional Clinical Viewer (ClinicalConnect™). The South West CCAC is recruiting for the position of:

Project Manager

Are you an effective leader passionate about achieving results? Join the South West CCAC as Project Manager responsible for the deployment of cSWO Program services across South West Ontario.

Reporting to the Program Lead and shared leadership, the Project Manager is responsible for providing assistance, coordinating, and reporting support to the Regional Program Lead and shared leadership. This involves implementing and managing project deliverables, researching and identifying any integration and implementation gaps as well as unique delivery strategies to mitigate risks.

The Project Manager is also responsible for assisting in the development and implementation of short and long-term strategies to enhance the delivery of the cSWO Program.

Qualifications and experience will include:

  • Post–secondary education in health sciences/business administration, information management and technology and/or equivalent education and experience.
  • 3-5 years of leadership/management experience.
  • Experience and knowledge of privacy and information management security in healthcare environments.
  • Ability to manage complex projects involving multiple agencies, budgets and stakeholders.
  • Ability to build teams and lead through system level change.
  • Ability to resolve conflicts and achieve consensus.
  • Time management skills and ability to prioritize multiple and conflicting demands.
  • Organizational skills and solid knowledge of project management and quality improvement methodology.
  • Knowledge of systems, resources and technology used in health care environments, especially Eclipse PPM software.
  • Knowledge of cSWO throughout the South West cluster area.
  • PMP designation or equivalent project management experience.
  • Proficiency in French is an asset.

The successful candidate will be employed by the South West CCAC on a contract basis for one year with the possibility of extension. Travel is required throughout the South West CCAC’s boundaries. Main office site to be negotiated.
