RN – Clinical Practice Manager
Summary of Position
Accountable for supervision and clinical support of nurses. Responsibilities include clinical education and support, performance management. Creation, implementation and sustainability of Clinical approach for Eastern Canada.
The Clinical Practice Manager reports directly to the Regional Director.
Scope of Practice
The Clinical Practice Manager must be a licensed Registered Nurse practicing within the Standards of the CRNNS and work in accordance with the Regulated Health Professional Act (RHPA).
The Clinical Practice Manager adheres to CBI/We Care Home Health’s vision, mission, objectives, standards, policies, procedures and protocols and where appropriate, the policies and procedures of Continuing Care, DHW and or other customers of CBI/We Care Home Health.
Ensuring the privacy of clients/caregivers/families, the health care team and fellow colleagues.
Safety Reporting any client safety concerns or evidence of client abuse or child abuse as required by law.
Reporting immediately any safety concerns as per CBI/We Care Home Health’s policies and procedures and the Occupational Health and Safety Act.
Participating in all mandatory Health and Safety activities.
Ensuring proper use of routine practices and use of personal protective equipment when required.
Working safely at all times according to CBI/We Care Home Health standards.
Other Identifying own learning needs and seeks out resources to meet those needs.
Maintaining professional growth and development through ongoing education.
Performing other tasks/responsibilities as assigned, other than those stated.
Position Specific Responsibilities Identifies and leads process for engaging in community based chronic health issues
Department Planning and Procedures
Engages with Community Stakeholders
Participates in the development of the Nursing/CCA Program policies, procedures and guidelines consistent with current professional and legal standards of practice
Implementation of nursing/CCA best practices with front line Care Givers
Develops, implements and evaluates Nursing/CCA Program training and education activities
Provides professional expertise related to clinical treatment and client services
Leads Nursing committees at local level
Participates in accreditation preparedness activities
Communication Facilitates effective communications strategies to ensure quality client care
Facilitates open lines of communication both internally and externally between teams and partnerships
Assists with the implementation of the Nursing/CCA Program’s electronic documentation and communication
Facilitates work flow and educates staff on Outcome Based Care initiatives
Quality and Risk Management: Ensures effective management of client, Continuing Care, DHW and nursing/CCA issues and concerns
Implementation of Risk Management activities within the Nursing/CCA Program
Participates in quality initiatives such as accreditation and best practices
Participates in data collection/ metrics to ensure ongoing quality
Participates in or liaise with PPRC, ensures collaboration
Supervision and Human Resources Assists with interviews for new staff when necessary
Assist with orientation and ongoing clinical education of nurses/CCAs
Performance management of nurses including coaching, supervision, joint visits, performance reviews, chart audits, and annual competency assessments
Shares supervisory on call responsibilities
Provides oversight to the Nursing/CCA Program for the Manager’s planned absences
Client Services Required to provide nursing services to clients in the community as needed
Customer Relationships Identify and develop opportunities for community-based partnerships
Participates on external committees and working groups as required
Communicates on a day to day basis with partners as necessary
Assists with communication of processes and procedures to nursing team
Qualifications/Requirements Graduate from an approved nursing program
Current registration in good standing with the CRNNS
Minimum 3 years experience in community health care
Nova Scotia wound care protocol training would be an asset
Experience in palliative care is an asset
Experience in infusion therapy is an asset
Experience in health care supervision or leadership preferred
Valid Nova Scotia drivers license, reliable vehicle. Travel is required throughout region.

Participation and completion of all Health and Safety requirements
Current CPR certification
Performance Monitoring Manager’s evaluation
Performance monitoring will be completed formally via a performance evaluation as required by CBI/We Care Home Health. This will facilitate continuous development and improvement of clinical and leadership practice. Performance monitoring will be evaluated utilizing, but not limited to:
