Community Health Coordinator Contract – 21 hours per week Summary Reporting directly to the Executive Director, the Community Health Coordinator (CHC) is responsible for developing and facilitating community planning initiatives; identifying and fostering development opportunities; and ensuring public input into development plans and projects as it relates to palliative care in the Burlington community. The CHC helps communities to bring about social change and improve the quality of life for palliative residents living in Burlington. The CHC works with individuals, families and the whole community to 1) identify their assets, needs, opportunities, right and responsibilities, 2) plan what is to be achieved and appropriate action items, 3) develop sustainable activities and services to generate aspiration and confidence. The Community Health Coordinator acts as a link between the community and various other local authorities and sector providers such as the police, social workers, local government, educators and healthcare services. The CHC seeks to actively engage communities in making sense of the palliative healthcare issues which affect their lives, setting goals for improvement and responding to the problems and needs through empowerment and active participation. In the course of their duties, the CHC will consistently demonstrate our core values of respect, honesty and equity. The CHC will honour confidentiality and the autonomy of the residents and staff. The Community Health Coordinator will demonstrate appropriate conflict resolution strategies and participate in reflective practice to identify challenges and contribute to solutions. Duties Position Requirements Additional Position Requirements (the ideal candidate will also possess the following professional qualities) Schedule of Hours This position requires a flexible schedule. It may be necessary to work after hours and weekends. The Community Health Coordinator will be required to work a maximum/averaged hours of 20 hours per week and is required to submit an invoice bi weekly. Working Conditions The Community Health Coordinator will be required to work at the Carpenter Hospice and from home. In both situations, there may be significant computer use in an office environment with exposure to frequent interruptions. Ability to lift 20 lbs repeatedly and without restrictions. Some travel required.
