Community Health Coordinator

Contract – 21 hours per week

Summary

Reporting directly to the Executive Director, the Community Health Coordinator (CHC) is responsible for developing and facilitating community planning initiatives; identifying and fostering development opportunities; and ensuring public input into development plans and projects as it relates to palliative care in the Burlington community. The CHC helps communities to bring about social change and improve the quality of life for palliative residents living in Burlington. The CHC works with individuals, families and the whole community to 1) identify their assets, needs, opportunities, right and responsibilities, 2) plan what is to be achieved and appropriate action items, 3) develop sustainable activities and services to generate aspiration and confidence. The Community Health Coordinator acts as a link between the community and various other local authorities and sector providers such as the police, social workers, local government, educators and healthcare services. The CHC seeks to actively engage communities in making sense of the palliative healthcare issues which affect their lives, setting goals for improvement and responding to the problems and needs through empowerment and active participation.

The Community Health Coordinator will demonstrate appropriate conflict resolution strategies and participate in reflective practice to identify challenges and contribute to solutions.

Duties

• Research and provide recommendations on Community Palliative Health Development opportunities by identifying community skills/resources, assets, challenges and needs
• Facilitate the Community Palliative Health Development planning process for Carpenter Hospice
• Facilitate the development of, and implement a Community Palliative Health Development plan including vision, goals and objectives
• Facilitate the development, implementation and monitoring of work plans, timelines, budgets, milestones, and deliverables for a Community Palliative Health Development Palliative program
• Engage the community to play an active role in developing strategic responses and community solutions
• Write, edit and format publications, presentations and reports related to assigned projects
• Assist in raising public awareness on palliative issues relevant to the community
• Represent Carpenter Hospice in any Community Palliative Development working groups, task forces and shared initiatives
• Liaison with Community stakeholders and partners including government representatives
• Research and analyze project impact, strategic value and “next generation” options
• Provide Executive Director with regular updates related to progress, milestones, potential and emerging challenges and any feedback from project partners and sponsors
• Develop action plans in alignment with the organization’s strategic direction
• Other duties as required

Position Requirements

• Bachelor’s degree required; preferably in Public Health, Social, Biological or Health Sciences, Education, Public Administration, or related discipline
• At least 2 years related professional experience
• Experience in palliative care
• Exceptional communication skills
• Experience in public health communications
• Expert in Microsoft Office products (Word, Outlook, Excel, PowerPoint)

Schedule of Hours

This position requires a flexible schedule. It may be necessary to work after hours and weekends. The Community Health Coordinator will be required to work a maximum/averaged hours of 20 hours per week and is required to submit an invoice bi weekly.

Working Conditions

The Community Health Coordinator will be required to work at the Carpenter Hospice and from home. In both situations, there may be significant computer use in an office environment with exposure to frequent interruptions. Ability to lift 20 lbs repeatedly and without restrictions. Some travel required.
