We are recruiting for a SUPPORT COORDINATOR (Adult Services)

- This is a contract full-time role (35 hours per week Monday to Friday 9am to 5pm) until September 2017 based at 147 Wyndham St. Guelph.

This role is responsible for providing recovery oriented support to individuals with serious mental health challenges and their families, including advocacy with and for individuals, collaboration with community partners and supports for issues dealing with finance, housing, activities of daily living, education and employment. A comfort in providing community based care is a must.

The incumbent requires:

  • Undergraduate training in psychology, social work or a relevant health or social science field
  • Minimum of three years’ experience providing community based mental health/addictions services
  • Experience with concurrent disorders is an asset
  • Demonstrated assessment and crisis intervention skills
  • Knowledge of or experience with motivational interviewing an asset
  • Problem-solving ability to identify problems, prioritize issues, generate possible solutions and evaluate the effectiveness of the implementation of these solutions
  • Ability to identify realistic, specific goals and objectives to address issues
  • In-depth knowledge of the Recovery philosophy
  • Effectiveness in a multi-disciplinary environment and ability to work collaboratively with other service partners
  • Demonstrated ability to work effectively with primary care and medical staff is an asset
  • Strong interpersonal and communication skills (verbal and written)
  • Demonstrated understanding and sensitivity to social and cultural diversity
  • Organizational skills and the ability to adapt to competing and changing priorities
  • Proficiency with MS Office, Outlook and various database software
  • A valid driver’s license and own transportation, as travel is required

CLASSIFICATION AND RATE OF PAY: Level 5 (OPSEU) – $27.14 to $34.32 per hour
