Job Summary: Working in collaboration with individuals experiencing mental illness, the Transitional Case Manager (TCM) will facilitate the transition from immediate mental health needs to an increased level of autonomy by promoting stability and empowerment on a short term basis (from four to six months). TCMs must also be able to develop strong working relationships and collaborative partnerships with other providers including but not limited to hospital, addiction services, social supports, etc. This is a full-time, six-month contract position, the successful candidate will receive a partial prescription drug benefit and the option of enrollment in the Healthcare of Ontario Pension Plan (HOOPP).

The successful candidate must have a Police Vulnerable Sector Check (PVSC) completed in the last six months. In order to avoid delays in beginning a position, interested applicants residing in London who are in need of a PVSC are encouraged to apply as PVSCs in the City of London are currently taking at least one month to be processed and offers of employment cannot be made without receipt of a PVSC.

Job Description and Primary Responsibilities:

  • Assisting with short-term case management by providing an urgent/emergent response to clients in need
  • Engaging with clients on a short-term basis to support their immediate needs (approximately 4-6 months)
  • Providing information and referrals
  • Conducting a variety of assessments (skills, functional ability, needs, barriers, etc.)
  • Empowering clients to identify personal goals and preferences to work towards recovery
  • Assisting clients to establish recovery-focused individualized rehabilitation plans
  • Supporting clients to navigate the mental health system and barriers they may encounter
  • Assisting and advocating for/with clients to obtain needed services and supports
  • Identifying, developing, and maintaining strong links with a wide range of community partners/resources
  • Completing required documentation as per agency and provincial case management standards
  • Other duties as necessary

Qualifications / Requirements:

  • Degree or diploma in a health or social science discipline
  • Knowledge of and experience working with individuals with a serious mental illness in a community setting
  • Excellent knowledge of community resources in London
  • Sound knowledge of computer skills and office technology (will be required to maintain notes on client database (Roxy), use Word, Outlook, internet searches and phone technology, etc.)
  • Valid Driver’s License and access to a vehicle to carry out job duties
  • Current clear Police Vulnerable Sector Check (PVSC)
  • First Aid, CPR, CPI, and ASIST training are an asset
  • Ability to attend work regularly

Hours of Work:

  • 35 hours per week
  • Hours will be predominantly Monday–Friday, 8:30 am–4:30 pm with one unpaid hour for lunch; occasionally may be required to work outside of those hours
