The Community Development Coordinator will provide strategic support for community development within the CRPCN region. This position is designed to provide ongoing support, advocacy, collaboration and coordination for community driven initiatives that support the establishment of healthy communities. This person is an employee of CRPCN and serves as a consultant and resource to both CRPCN staff and the community involved in strengthening community action.
Knowledge and Skills
- Broad understanding of health promotion with emphasis on strengthening community capacity
- Ability to assist with community assessments
- Broad knowledge of group process and experience in community engagement
- Applied project management, program planning and evaluation skills
- Demonstrated skills in consultation, coaching and education processes
- Ability to work effectively in team and community settings
- Demonstrated leadership skills
- Excellent communication skills, both oral and written
- Computer skills and knowledge of Microsoft Office
- Ability to be flexible in hours of work; * Ability to prioritize workload and to work independently
- Strong facilitation and networking skills are assets
- Willing to travel
Expectations
- Develop goals and objectives, plan, and evaluate health promotions services for the communities
- Initiate, develop, coordinate, implement and/or lead and evaluate initiatives at the local level through collaborative community relationships.
- Maintain a strong relationship with CRPCN Clinical Innovation Managers, Evaluation team and other PCN staff (at the local medical clinics).
- Establish positive relationships with community organizations and other sectors through activities such as partner development, public participation, linking of initiatives between sectors, championing the building of community capacity and assisting outcome measurement of improved health and well-being.
- Collaborate with internal and external stakeholders
- Be an active participant in the CRPCN’s community of practice
- Represent CRPCN in a professional manner
- Collate and review data to facilitate the application of community development
- Conduct environmental scans of resources to assist with the implementation of community development
- Prepare background documentation and reports based on research findings and literature to support the advancement of community development
- Develop project proposals for CRPCN funding and assist with proposals for other grant funders
- Participate in the development of strategic plans for the communities within the CRPCN
Education and Experience
- Undergraduate degree in a relevant health sciences program with emphasis on health promotion preferred
- 5 – 7 years’ experience in a community/public health setting
