POSITION: CASUAL PART TIME – SPIRITUAL CARE PRACTITIONER DEPARTMENT: SPIRITUAL CARE – ACUTE AND TRANSITIONAL CARE VALUE STREAM POSTING DATE: DECEMBER 2, 2014

SCOPE: Reporting to the Value Stream Group Leader, Acute and Transitional Care this Spiritual Care Practioner is an adjunct role to provide spiritual care coverage in the absence of the staff chaplain. The Spiritual Care Practitioner is responsible for the offering of spiritual and religious care to patients, family members / significant others, volunteers and staff especially where crisis, decision making, and difficult situations arise while in Hospital. The Spiritual Care Practitioner facilitates resources in collaboration with other health care providers and community clergy / religious leaders. This position functions as a member of the interdisciplinary healthcare team in the delivery of patient care. PRIMARY RESPONSIBILITIES:

 Review patient lists/requests daily; Conduct daily rounds

 Provide spiritual counseling to patients, families and significant others

 Facilitate spiritual, multi faith, cultural practice to promote healing and well being

 Provide support/counseling to staff during crisis situations or where appropriate

 Respond to after-hours on call 24/7 referrals

 Communicate with community clergy to provide pastoral services

 Work with patient/family to support acceptance of disability or death when recovery is not possible

 Provide bereavement support QUALIFICATIONS:

 Endorsed through community of religious affiliation

 Secondary School Graduate, Undergraduate Degree, Graduate degree preferred

 Completion of at least one Basic Unit of Clinical Pastoral Education through the Canadian Association for Spiritual Care (CASC), or equivalent training/experience

 Current membership in CASC

 Experience in an acute and complex continuing care settings and psychiatric pastoral counseling preferred

 Excellent communication, interpersonal, critical thinking and time management skills

 Excellent customer service skills

 Demonstrated leadership ability

 Excellent attendance record

 Demonstrated ability to work both as, a member of a multidisciplinary team and an independent professional

 Demonstrated proficiency in Microsoft Office: Word, Excel, PowerPoint, Outlook
