SCHEDULING:

Contract, full-time 37.5 hours per week- flexible evenings and weekends (ending March 31, 2016) with possible renewal based on funding.

JOB PURPOSE & GENERAL ACCOUNTABILITY:

The client care coordinator will assist clients and families to navigate the health and human services’ sector. The coordinator will assess and support the client and family to be matched to appropriate resources. The coordinator will provide volunteers coaching and on-going support to assist them with client and family encounters.

SCOPE OF ROLE:

  • Support the development of client and family care planning
  • Provide information and referral to other health and human service providers that can help to support the care plan
  • Support groups (examples: bereavement, sharing circles (debrief) and compassion fatigue support)
  • Participate in community outreach and education

QUALIFICATIONS:

Experience and Competency

  • Client and family centered approach to care planning
  • Firm understanding of the hospice palliative care sector and other related community services
  • Familiar with community served (Caledon, Bolton and Brampton)
  • Ability to motivate, coach and support volunteer teams

The ideal candidate possesses the following:

Education

  • University degree or equivalent education and/or experience in social services
  • Minimum 3 yrs. experience working in a not-for-profit – health or human service sector
  • Experience in supportive counseling, group facilitation and clinical interventions with clients and families
  • Experience working with volunteers
  • Proficiency using databases, Microsoft programs and internet applications
  • Knowledge of current trends, issues and practices in hospice palliative care an asset
  • Access to a reliable automobile and wiliness to travel within Caledon, Bolton and Brampton
  • Excellent customer service skills and the ability to deliver exceptional customer service within the community
  • Excellent written and verbal communication skills
