Reporting to the Associate Manager of the Calgary TB Clinic, this position will have the primary responsibility of assessing TB clients, developing intervention plans, addressing treatment barriers and leading TB social work-specific clinical consultation, quality improvement and evaluation activities. In addition to the key responsibilities outlined in the provincial job description, the following activities are also applicable to this position.
Performing comprehensive and TB-specific psychosocial assessments with a patient/family centered approach and biopsychosocial-spiritual model of care
Offer advanced discipline-specific, proactive and effective communication with the patient/family and interprofessional care team members, in order to support coordinated and comprehensive patient/family centered, team-based, TB-specific care
Lead the development of TB-specific, patient/family-defined, social work measures that address barriers and challenges specific to TB treatment uptake and completion
Co-lead the development of TB program-defined effectiveness and optimization metrics for incorporation of social work into the TB program
Develop recommendations to program leadership, through the course of the position term, in regards to care optimization from a biopsychosocial-spiritual and discipline-specific perspective.
Qualifications:
Formal Education:
– Master of Social Work degree (equivalent education or experience may be considered)
Professional Membership:
– Registration with the Alberta College of Social Workers (ACSW)
Professional Experience:
– Two to five years of clinical experience with chronic or communicable-disease management preferred.
